Mixed lineage leukemia (MLL) gene rearrangements are a recurrent cytogenetic aberration in various types of hematologic malignancies. Although most MLL rearrangements are associated with well-defined breakpoints, some breakpoints do not match the established clonal rearrangements, most likely due to inversion events. In the present study, we searched for cryptic MLL rearrangements in bone marrow (BM) aspirates from patients with various types of hematologic malignancies using a multiplex polymerase chain reaction (PCR) and subsequent direct sequencing (SD). The analysis included a total of 77 BM aspirates from unselected patients with different hematologic malignancies and with a clonal karyotype at the time of diagnosis. A total of 29 (38%) BM aspirates showed an inversion of the MLL gene at 17q25.3 or 17q21.31, either in the absence (18 aspirates) or presence (11 aspirates) of an MLL rearrangement.
